Monthly Membership Pricing

Ages 18 and up $89 per month
Ages 3-17 with adult parent or guardian membership $49 per month

Pricing is for ages 3-17 years old when an adult parent or guardian is also enrolled
Ages 3-17 without adult parent or guardian membership $89 per month

If no adult parent or guardian is enrolled with the child, then child membership is $89/month
Family memberships with up to 2 adult and 3 child members on a single account $300 per month

Additional adults can be added at $89/month and children ages 3-17 at $49/month

Home visits are available within a 7 mile radius of the clinic for an additional $150/visit. If you decide to cancel, it is simple, just let us know. We do ask for 30 days notice. If you choose to leave and later wish to return, a $178 re-enrollment fee applies. This helps us prioritize our current members and maintain consistent care. Re-enrollment is subject to current practice capacity.

Simple Pricing Structure

Patients are automatically billed on a monthly basis and invoiced prior to billing.
There is no cost or obligation for an introductory meeting to determine if Cool Creek Family Health is right for you.
No contract is required and you can cancel at any time if you feel like the clinic is not the right fit for you.
There are never any copays for visits or communication, however there may be additional cost for certain procedures, labs, and prescriptions. These costs will be passed to you at a greatly reduced rate and you will be notified ahead of time if any cost will be passed to you.
To make this model work, we do not accept direct payment for one time visits, but rather we require monthly memberships.

While you will be blown away at how many services we include in your membership, here are a few things for which we do not provide service:

Initial evaluation and diagnosis for ADD/ADHD – We can help refer you to a neuropsychologist for appropriate testing. If you have had testing done and documentation confirming diagnosis, we are happy to help manage your ADHD medications along with the rest of your primary care.
Pain or Complex Psychiatric Management – We do not prescribe controlled substances (e.g. narcotics, benzodiazepines) for long term management. We are happy to help coordinate your referral to a pain management or psychiatric specialist for those prescription needs.
Fracture Care – We can assess and order X-rays and diagnostics and can refer you to orthopedics for your fracture care.
