I believe that every member of our community deserves access to excellent healthcare. After several years of working in insurance-based primary care clinics, I became frustrated with never getting as much time as I would have liked to spend with my patients. Variations in insurance coverage from one patient to the next caused limitations dictating how I could care for my patients. Pressure from health care administrators to see as many patients as possible limited my time, preventing thorough and adequate follow up. On a daily basis, I see primary care become more and more fragmented with a minimal level of personal connection.

I started this Direct Primary Care clinic in Carmel, Indiana to be able to provide more comprehensive and personalized primary care. I believe patients and providers alike deserve a personal connection with one another in order to foster a healthcare partnership. With the Direct Primary Care model, I can fully focus on your needs and build a lasting relationship with you without any limitations or outside pressures.

The nursing model of healthcare focuses on getting to know the patient as a sum of a whole system of parts. As a nurse practitioner with my own DPC clinic, I want to take primary care back to its roots with the main focus being on you — my patient. Personalized primary care is my goal for you and your family.

I am a board-certified Family Nurse Practitioner and have been a proud Hoosier since birth. I was born and raised in Columbus, IN and received my Bachelors’ in Biology from Indiana University. After a few years in a corporate setting, I earned my Master’s in Nursing from DePaul University. Then, I received my Family Nurse Practitioner certification from Frontier Nursing University. Throughout my nursing career, I have practiced in the hospital, urgent care and family practice settings.​

I live in Carmel with my husband and three children. I love cooking, especially over an open campfire. Also, I enjoy riding my spin bike, working in my garden/yard, practicing yoga and playing tennis. But most importantly, I love meeting new people, building lasting relationships, helping my community, and putting smiles on your face.

My background is in helping management teams and operational units get and use the business intelligence necessary to make strategic decisions. I’ve had roles within publishing and higher-ed and now healthcare with Cool Creek Family Health where I manage the budgeting process, financial projections and operational aspects of the business. I earned my MBA in Finance from DePaul University in 2012 and my BS from Indiana University’s Kelley School of Business back in 2003.

A Cheesehead by birth but raised a Hoosier in Elkhart, I love Indiana and am thankful life brought us back here. I love our four seasons and appreciate the lack of lake effect snow in the central part of the state compared to up north. I love playing tennis with other beginners and enjoy family bike rides to downtown Carmel, Indiana.

Dr. Jessica Saberman was born and raised in Chicago, then attended Colorado College for undergrad and Mayo Medical School in Minnesota for her medical degree. She then went on to University of Minnesota – Minneapolis for Family Medicine residency, and while in Minneapolis she met her husband David. Upon completion of residency in 2001, together they moved to the Indianapolis area where Dr. Saberman provided full spectrum primary care for over 21 years, including prenatal care and delivery, care of infants and children and young families through end of life care. She worked with St. Vincent Primary Care Network in Fishers for almost 10 years, then moved to IU Health Primary Care in Fishers and Noblesville for 11 years.  In April 2023 she “hung up her stethoscope” and now works in a 100% virtual primary care practice from the comfort of her home. Farah and Dr. Saberman first met in 2016 while working together with IU Health in Noblesville, and they have enjoyed a rewarding professional and personal relationship ever since. When not working, Dr. Saberman enjoys reading, cooking and baking, gardening, and time spent curled up on the couch with her husband and pets.

Why the name Cool Creek Family Health?

Cool Creek connects many parks in our area and Farah and Andy love visiting each of them with their family. The creek is a great place to cool off on a hot day, fish, or hike and spot the birds and other wildlife. Cool Creek played such a role in their family and was such a breath of fresh air to visit after living in a larger city that it was a natural fit for our business name. We hope Cool Creek Family Health is a breath of fresh air for our members as we travel their healthcare journey together.

A family nurse practitioner (FNP) is a highly trained nurse and licensed healthcare professional who specializes in providing primary and preventive care to individuals of all ages, from infants to elderly patients. FNPs are advanced practice registered nurses (APRNs) with specialized education and clinical training that enables them to diagnose illnesses, develop treatment plans, prescribe medications, and offer holistic healthcare services. They focus on promoting overall wellness, managing chronic conditions, conducting physical exams, and emphasizing patient education. While FNPs collaborate closely with physicians and other healthcare professionals, they are capable of independently delivering comprehensive healthcare services, making them valuable primary care providers in various healthcare settings, including outpatient clinics and community health centers. Farah Myers and the healthcare providers at Cool Creek Family Health are FNPs and look forward to learning more about your healthcare journey.

Direct Primary Care (DPC) is a model for primary care practice that takes the insurance and health administrators out of the exam room. For a monthly membership fee for our all-inclusive services, we spend more time with you and less time with paperwork. You get more comprehensive, personalized primary care because the main focus is you!

We provide full service primary care. In addition to direct access to your provider, extended appointment times, no wait times, and transparent pricing, we include several services in your membership. Within your membership fee, we include a set of annual labs, in house testing (urine dip, strep, flu, mono), various procedures (ear wax removal, wart cryotherapy, skin biopsy, etc.), and EKG. We do our best to keep all your cost within your membership fee. If you do require services not included in your membership (ie. imaging, biopsy analysis, etc.), you are given a clear price through our contracted discount rates. You never walk out the door without knowing the cost of your healthcare. We guarantee no surprise bills and strive to avoid added cost to your membership. We are incentivized to keep you healthy AND happy!

No. Your membership is month-to-month. Of course we hope to establish a long term relationship as your health care provider! If you find that this model or practice is not for you, you can cancel with 30 days written notice.

You can still use your insurance to get your prescriptions. However, we also have contracted with pharmacies for discounted rates. We work with you to find the best pricing for your prescriptions.

We are able to provide controlled substances for acute (short term) management. However, we do not provide prescription management of controlled substances for any chronic conditions. This includes but is not limited to sleep, mood disorders and chronic pain.

We are available between regular business hours of 8am-430pm for in office visits, video visits, and phone consults. you can reach us via text or email after hours and by phone or video visit for urgent needs. We also offer house calls within 7 miles of the clinic for an additional fee. If it is a true emergency, please call 911 or go to your nearest emergency room.

Traditional insurance-based primary care providers have 2500-3500 patients. We limit ours to a fraction of this per provider to ensure you with individualized, comprehensive and high-quality care. This gives Farah time to know each patient personally and develop quality relationships with her patients.

Our providers services are not dependent on insurance. We removed health insurance from exam room so your healthcare relationship is strictly between you and your provider. Because we are not hassled by insurance for our services, we provide high quality services. We do not bill insurance for our provider services or provide superbills for insurance reimbursement. However, your insurance can be billed for larger costs such as speciality labs, imaging, specialist visits, vaccines, and other outside services. Pending your HSA/FSA company, our fees can be compliant with HSA/FSA accounts as memberships and be a qualified medical expense. You should double check with your benefits company. However, we do recommend paying annually or semi-annually as the best practice in order to use these accounts.

Yes. DPC is a primary care medical service and is not considered any type of insurance. It is recommended that you still carry medical insurance (catastrophic care) or health share coverage to cover any unforeseen events as well as non-routine labs, some medications, specialist visits, emergency care, hospitalizations and surgery. If you need insurance, we can help introduce you to some great resources that we trust.

Right now, you already do! You pay an insurance premium and then any further fees up until you meet your deductible. As a DPC member, rather than pay higher insurance based rates towards a deductible you may never meet, you pay a flat rate for all your primary care services. There are no copays, surprise bills, hidden fees, or unknown extra charges. Your membership covers annual labs, in-house testing and procedures as well as comprehensive consultation and follow-up care. If you have not met your deductible and we do not provide a necessary service, we can help to find you the best prices for that service and help coordinate your care with outside services.
