Meet Farah Myers, FNP, DipACLM

As the founder and owner of Cool Creek Family Health, I’m passionate about providing a more personal, connected, and transparent model of care through direct primary care in Carmel, Indiana. After years of practicing in traditional, insurance-driven clinics, I saw how rushed visits, administrative pressures, and inconsistent coverage often got in the way of real, meaningful healthcare. Patients were being reduced to numbers. Providers were burnt out. And the system as a whole had become more reactive than relational.

I created this practice as an alternative—one rooted in a direct relationship and a strong belief that patients deserve time, access, and thoughtful care from someone who truly knows them. By stepping away from the insurance treadmill, I can focus entirely on your health, your goals, and your story. I’m a board-certified Family Nurse Practitioner, born and raised in Columbus, Indiana. I hold a bachelor’s degree in Biology from Indiana University, a Master’s in Nursing from DePaul University, and completed my Family Nurse Practitioner training at Frontier Nursing University. Over the years, I’ve cared for patients in hospitals, urgent care centers, and family practice settings.

In 2024, I earned the distinction of Diplomate of the American College of Lifestyle Medicine, deepening my expertise in evidence-based approaches that treat and reverse chronic disease through nutrition, movement, stress reduction, and sustainable behavior change. It’s a privilege to help my patients build healthy, fulfilling lives with longevity and vitality in mind.

At home in Carmel with my husband and three children, I enjoy cooking over a campfire, long walks, weight training, gardening, yoga, and tennis. Most of all, I love meeting new people, building long-lasting connections, and making sure every patient feels seen, supported, and cared for.

Hi, I’m Mikayla Hare, a board-certified Family Nurse Practitioner with a deep commitment to holistic, patient-centered care. My approach is rooted in listening closely, building genuine relationships, and empowering patients to take charge of their health—because I believe healthcare should be a partnership, not a checklist.

Originally from Tennessee, I’ve spent the last several years in the beautiful Bluegrass state of Kentucky before settling here in Carmel with my husband. I earned my Bachelor of Science in Nursing from Trevecca Nazarene University and later completed my Doctorate of Nursing Practice at Belmont University. I’m certified through the American Nurses Credentialing Center to care for patients across the lifespan, and I love the variety that family medicine offers.

My nursing journey began on the Mother-Baby unit, where I developed a strong foundation in women’s health and a passion for supporting women through all stages of life. I later expanded my experience in urgent care, treating a wide range of acute conditions. Most recently, I’ve devoted the past several years to working closely with women, particularly those navigating perimenopause, hormone imbalances, and lifestyle transitions.

At our Direct Primary Care practice, I specialize in hormone replacement therapy (HRT) for men and women, perimenopause management, lifestyle medicine, and holistic care. My goal is to create individualized care plans that support the whole person—physically, mentally, and emotionally. Whether you’re managing chronic symptoms or seeking more balance in your life, I’m here to walk alongside you with empathy, education, and evidence-based strategies.

Outside the clinic, I enjoy spending time outdoors with my husband, being active in our church, and staying connected with family and friends. As a former collegiate golfer, I still love the thrill of competition and bring that same passion and focus to my work in healthcare.

Meet Andy Myers

Director of Finance & Operations

As the business manager at Cool Creek Family Health, I’m proud to support our mission of delivering outstanding Direct Primary Care in Carmel, Indiana. With a background in business intelligence and strategic operations, I love being part of a team that provides an exceptional concierge healthcare alternative—especially for small businesses looking for better employer healthcare solutions. Working alongside our talented family nurse practitioner colleagues, I help ensure the practice runs smoothly by overseeing budgeting, financial forecasting, and operational planning.

Before joining Cool Creek, I worked in publishing and higher education, helping leadership teams make data-informed decisions. I earned my MBA in Finance from DePaul University in 2012 and my BS from Indiana University’s Kelley School of Business in 2003.

Although I was born in Wisconsin, I grew up in Elkhart and am grateful that life brought our family back to this great state. I love Indiana’s four seasons, especially hiking through our oak-hickory forests, skiing in winter (I love snow however, I don’t miss the northern lake-effect snow!), and biking with my family to downtown Carmel, Indiana. You’ll often find me enjoying tennis on our numerous local courts or sharing meals with my family after a day outside.

Meet Dr. Jessica Saberman

Collaborating Physician

Dr. Jessica Saberman was born and raised in Chicago, then attended Colorado College for undergrad and Mayo Medical School in Minnesota for her medical degree. She then went on to University of Minnesota – Minneapolis for Family Medicine residency, and while in Minneapolis she met her husband David. Upon completion of residency in 2001, together they moved to the Indianapolis area where Dr. Saberman provided full spectrum primary care for over 21 years, including prenatal care and delivery, care of infants and children and young families through end of life care. She worked with St. Vincent Primary Care Network in Fishers for almost 10 years, then moved to IU Health Primary Care in Fishers and Noblesville for 11 years.  In April 2023 she “hung up her stethoscope” and now works in a 100% virtual primary care practice from the comfort of her home. Farah Myers, FNP, DipACLM and Dr. Saberman first met in 2016 while working together with IU Health in Noblesville, and they have enjoyed a rewarding professional and personal relationship ever since. When not working, Dr. Saberman enjoys reading, cooking and baking, gardening, and time spent curled up on the couch with her husband and pets.

Why the name Cool Creek Family Health?

Cool Creek connects many parks in our area and Farah Myers FNP, DipACLM and Andy Myers love visiting each of them with their family. The creek is a great place to cool off on a hot day, fish, or hike and spot the birds and other wildlife. Cool Creek played such a role in their family and was such a breath of fresh air to visit after living in a larger city that it was a natural fit for our business name. We hope Cool Creek Family Health is a breath of fresh air for our members as we travel their healthcare journey together.

As your family nurse practitioner, I recently earned the DipACLM certification, which stands for Diplomate of the American College of Lifestyle Medicine. This certification signifies that I have specialized training in using evidence-based lifestyle changes to prevent, treat, and often reverse chronic diseases.

Direct Primary Care (DPC) is a model for primary care practice that takes the insurance and health administrators out of the exam room. For a monthly membership fee for our all-inclusive services, we spend more time with you and less time with paperwork. You get more comprehensive, personalized primary care because the main focus is you!

We provide full service primary care. In addition to direct access to your family nurse practitioner, extended appointment times, no wait times, and transparent pricing, we include several services in your membership. Within your membership fee, we include a set of annual labs, in house testing (urine dip, strep, flu, mono), various procedures (ear wax removal, wart cryotherapy, skin biopsy, etc.), and EKG. We do our best to keep all your cost within your membership fee. If you do require services not included in your membership (ie. imaging, biopsy analysis, etc.), you are given a clear price through our contracted discount rates. You never walk out the door without knowing the cost of your healthcare. We guarantee no surprise bills and strive to avoid added cost to your membership. We are incentivized to keep you healthy AND happy!

No. Your membership is month-to-month. Of course we hope to establish a long term relationship as your family nurse practitioner! If you find that this model or practice is not for you, you can cancel with 30 days written notice.

You can still use your insurance to get your prescriptions. However, we also offer some commonly used generics in-office if we can offer them at a competitive rate. We work with you to find the best pricing for your prescriptions.

We are able to provide controlled substances for acute (short term) management. However, we do not provide prescription management of controlled substances for any chronic conditions. This includes but is not limited to sleep, mood disorders and chronic pain.

We are available between regular business hours of 8am-430pm for in office visits, video visits, and phone consults. you can reach us via text or email after hours and by phone or video visit for urgent needs. We also offer house calls within 7 miles of the clinic for an additional fee. If it is a true emergency, please call 911 or go to your nearest emergency room.

Traditional insurance-based primary care providers (PCPs) have 2500-3500 patients. We limit ours to a fraction of this per PCP to ensure you with individualized, comprehensive and high-quality care. This gives Farah Myers, FNP, DipACLM and Mikayla Hare, DNP, APRN, FNP-BC time to know each patient personally and develop quality relationships with their patients.

Our healthcare provider services by our family nurse practitioners are not dependent on insurance. We removed health insurance from exam room so your healthcare relationship is strictly between you and your nurse practitioner. Because we are not hassled by insurance for our services, we provide high quality services. We do not bill insurance for our nurse practitioner services or provide superbills for insurance reimbursement. However, your insurance can be billed for larger costs such as speciality labs, imaging, specialist visits, vaccines, and other outside services. Many of our patients choose to use their HSA cards to cover their membership costs for themselves and their families. Direct primary care is an eligible expense for HSAs.

Yes. DPC is a primary care healthcare service and is not considered any type of insurance. It is recommended that you still carry medical insurance (catastrophic care) or health share coverage to cover any unforeseen events as well as non-routine labs, some medications, specialist visits, emergency care, hospitalizations and surgery. If you need insurance, we can help introduce you to some great resources that we trust.

Right now, you already do! You pay an insurance premium and then any further fees up until you meet your deductible. As a DPC member, rather than pay higher insurance based rates towards a deductible you may never meet, you pay a flat rate for all your primary care services, reducing your downstream claims costs. There are no copays, surprise bills, hidden fees, or unknown extra charges. Your membership covers annual labs, in-house testing and procedures as well as comprehensive consultation and follow-up care. If you have not met your deductible and we do not provide a necessary service, we can help to find you the best prices for that service and help coordinate your care with outside services.
